AYSO SECTION 2 TOURNAMENT

U10, U12 & U14

INSTRUCTIONS ON HOW TO COMPLETE PLAYER IDENTIFICATION CARDS FOR TOURNAMENT

There are two main ways to create player cards for tournament play.

1. Using the Affinity Platform: This method will insure that all info on player card will match player roster and insurance forms.

2. Open this PowerPoint document.  Document will most likely be in Protected View, click the Enable Editing button to add the information. Or these fillable versions can be used:  Player ID PDF, Coach ID PDF, Back PDF
Complete the following information on the card:

  • Name of player (must match roster and player application form)

  • AYSO ID Number (must match roster and player application form)

  • DOB – Date of Birth (must match roster and player application form)

  • Gender

  • Region no. (Region they are registered in)

  • Division (U10, U12 or U14)

  • Season (should be current fall season.  For example:  “Fall 2023”)

  • Blank spot for picture (either cut and paste a digital picture or glue in a passport size picture)

  • Laminate (Picture underneath the laminate)

IF USING ONE OF THE NON-ASSOCIATION PLATFORM FORMATS, DOWNLOACD, COMPLETE AND PRINT PAGE ONE - WITH THE PICTURES, THEN DOWNLOAD THE BACK, FLIP THE PAPER AND PRINT THE BACK WITH THE AYSO LOGO. YOU WILL NEED TO PRINT ONE SIDE AT A TIME UNLESS YOUR PRINTER CAN AUTOMATICALLY PRINT ON BOTH SIDES OF THE PAPER.

NOTE:  All information on the Roster, Player Application Form and Player Identification Card must match.
